It may be the last smile you ever see.

Smile–a new horror film from writer/director Parker Finn–lands in theaters at the end of the week, and one final, chilling trailer has dropped to wipe that smile off your face. Check out the video on this page.

The R-rated thriller–set for the big screen Friday, Sept. 30–is adapted from Finn’s short film Laura Hasn’t Slept, which took home the SXSW Film Festival’s special jury recognition Midnight Award.

In Smile, Dr. Rose Cotter (Sosie Bacon) experiences “frightening occurrences that she can’t explain after witnessing a bizarre, traumatic incident involving a patient,” according to the synopsis.

As things worsen, Cotter must confront “her troubling past in order to survive and escape her horrifying new reality.”

The film also stars genre standouts Jesse T. Usher (The Boys), Kal Penn (The Ashram), Rob Morgan (Stranger Things), Kyle Gallner (this year’s Scream) and Caitlin Stasey (Fear, Inc.).
